In our facility, interprofessional collaboration is promoted in various ways during patient transition. The care team, which consists of a physician, nurse, pharmacist, social worker, and any other appropriate healthcare professional, communicates, teams up, and interrelates. The care team takes steps right from the beginning such as educating the patient and their family on self-care, how to follow the care plan, and how to manage the medication.
